Health Action Plans
•MUST be completed by parent AND doctor if any history of food allergies, asthma, or seizures
•Please note clearly any emergency medications/interventions needed at school
•Medications kept at school for emergencies include inhaler, spacer, nebulizer, Benadryl/anti-histamine, Epi Pen, Diastat/seizure medication
•If student has diagnosis but no treatment needed, please fill out action plan accordingly so on file for reference (parent may do this, no doctor signature needed)
